Aging and Disability Resource Connection (ADRC)

California’s ADRC is a core partnership between Independent Living Center (ILC) and Area Agencies on Aging (AAA) and includes other extended partner organizations. ADRC’s Program is intended to act as a “NO Wrong Door” System which enables people ages 65 and older to connect with other ADRC partner organization to access a wide array of Long-Term Services and Support (LTSS) options in the community.

Expected Services:

• Enhanced Information and Referral
• Person-Centered Options Counseling
• Short-Term Service Coordination
• Transitions from Long-Term Nursing Facility to Community
